Due to the excessive amounts of spam that was entered in this form I was wondering about the following:

why would I implement captha on an internal app?
What f* lowlife would be interested in posting spam in an app this small?

Anyway. SF staff has in its ultimate wisdom decided to chmod 000 and chown root all 5 php files of this small application. I think they are right. This things works and has been working in our organisation for 3 years now. I guess it is bug free and I know it is easy to use. My advice: give it a try!
